17 alpha-hydroxyprogesterone, often abbreviated as 17-OHP, is a naturally occurring steroid hormone and a key intermediate in the biosynthesis of glucocorticoids, androgens, and estrogens. Structurally, it is a derivative of progesterone characterized by a hydroxyl group at the 17th carbon position, which plays a crucial role in its function and metabolism. In clinical contexts, 17-OHP serves as an important biomarker for diagnosing congenital adrenal hyperplasia (CAH), a genetic disorder affecting adrenal steroid synthesis. Elevated levels of 17-OHP in newborns are indicative of CAH due to 21-hydroxylase deficiency, leading to increased androgen production and the potential for abnormal sexual development. Measurement of 17-OHP is routinely performed in neonatal screening programs to promptly identify and manage this condition. Research into 17-OHP continues to advance understanding of steroidogenesis and its implications in various endocrine disorders, while also exploring therapeutic applications of synthetic analogs for reproductive health.
