Bupivacaine is a potent local anesthetic belonging to the amide group, widely utilized for regional, epidural, spinal anesthesia, and local infiltration. It is known for its long duration of action, often administered via spinal injection prior to surgeries like total hip arthroplasty, or injected into surgical wound sites to alleviate pain for up to 20 hours post-surgery. Bupivacaine functions by blocking the generation of action potentials in nerve fibers, thereby preventing pain signal transmission. Recent studies have explored its use in multivesicular liposome formulations to enhance pain relief and reduce opioid consumption post-musculoskeletal surgery. However, continuous joint infusion of bupivacaine can lead to chondrotoxicity, necessitating careful administration strategies.
