Fluconazole is a triazole antifungal medication predominantly used to treat and prevent fungal infections such as candidiasis, cryptococcal meningitis, and other systemic fungal infections. Its chemical structure features a triazole ring that confers specificity in inhibiting the fungal cytochrome P450 enzyme 14-alpha demethylase, leading to impaired ergosterol synthesis, a crucial component of fungal cell membranes. Available for oral and intravenous administration, fluconazole is characterized by high oral bioavailability and good penetration into body fluids and tissues, including the cerebrospinal fluid, which enhances its utility in treating central nervous system infections. While fluconazole is generally well-tolerated, potential side effects include liver enzyme elevations and QT prolongation, necessitating monitoring in patients with pre-existing hepatic or cardiac conditions. Continuous research evaluates fluconazole's long-term efficacy and safety, particularly in contexts involving resistant strains or complex infections requiring combination therapy.
