HLA-DQA1, HLA-DPA1, and HLA-DRB1 encode key alpha and beta chain components of the human leukocyte antigen (HLA) class II system, which presents extracellularly derived peptides to CD4+ T helper cells and orchestrates adaptive immune responses. HLA-DQA1 pairs with HLA-DQB1 to form HLA-DQ, HLA-DPA1 pairs with HLA-DPB1 to form HLA-DP, and HLA-DRB1 pairs with HLA-DRA to form HLA-DR; among these, HLA-DRB1 is the most polymorphic class II gene and a major determinant of peptide-binding specificity. Extensive allelic diversity across these loci underlies interindividual variation in antigen presentation, influences susceptibility to autoimmune diseases (e.g., type 1 diabetes, celiac disease, rheumatoid arthritis), infection outcomes, and responses to vaccines, and is central to donor-recipient matching in transplantation. Expression is predominantly on professional antigen-presenting cells (dendritic cells, B cells, macrophages) and is induced by interferon-y via the CIITA transactivator. Structural variation and specific epitope motifs (such as the DRB1 "shared epitope") modulate T-cell tolerance and pathogenic T-cell activation, while linkage disequilibrium across the class II region forms common haplotypes with combined functional effects.
