Phenobarbital is a long-acting barbiturate used primarily as an anticonvulsant and sedative-hypnotic medication. It is one of the oldest and most commonly prescribed antiepileptic drugs, effective in managing various types of seizures, including generalized tonic-clonic and partial seizures. Phenobarbital works by enhancing the inhibitory actions of gamma-aminobutyric acid (GABA) on neurotransmission, thereby stabilizing neuronal activity in the brain. Its use extends to the treatment of status epilepticus and febrile seizures in children, although newer antiepileptic drugs have largely replaced it due to concerns about side effects and potential for dependence. As a barbiturate, phenobarbital can induce sedation, dizziness, and cognitive impairment. It also carries a risk for tolerance and addiction, necessitating careful dose management and monitoring. Despite these limitations, phenobarbital remains a valuable option in certain clinical scenarios, particularly in resource-limited settings due to its low cost and broad efficacy.
